Camp Forms

Most camp forms are filled out online at the time of registration.  You can always return later to make updates, but the deadline is 14 days before the start of the session being attended.

The main forms that require extra time and legwork have to do with medical information. For most campers, you just need current vaccination information and health history — and we require that every camper has seen a doctor within 24 months of attending camp. For children with prescription medications and / or medical issues that require our attention at camp, we require a health exam within 12 months of attending camp, signed by the physician.

Please note that there is a charge for late paperwork. We need the time to make sure that we are ready for your child.
